Sheltie Rescue of Utah Documents

As a rescue group, SRU requires a paper trail of documentation for event that involves a transfer of a dog (as well as for other events). The two most important pieces of such documentation are:

  • Relinquishment Form (available in both Microsoft Word format and PDF format) — We require that every owner who wishes to turn a dog over to Sheltie Rescue of Utah complete, sign, and date this form. This guarantees that there are no ambiguities about whether or not the dog was legally transferred to SRU. (This has saved our bacon several times; and the one time we failed to get a completed relinquishment form, the dog was reclaimed by the relinquishing party and ended up being treated very poorly. We learned our lesson.)
  • Adoption Contract (available in both Microsoft Word format and PDF format) — Whenever a dog is adopted from or through Sheltie Rescue of Utah, the adopting party or parties are required to complete, sign, and date this contract. We are very candid about the fact that the contract is intended to protect the dog — we are, after all, in the business of rescuing dogs, not putting them into jeopardy. There are a number of specific provisions that the adopting party is required to initial individually, certifying that they have read and understood each of those provisions.

There are, naturally, other documents that are central to an organization such as Sheltie Rescue of Utah. We believe in openness and transparency, and will make every effort to fulfill that goal. In order to provide all the information that we are legally allowed to provide to the public, we will post relevant documents on this web page. Those include such things as the Articles of Incorporation, the By-Laws, and financial statements. As soon as we are able to do so, we will make these available.
